Vitamin B6 (Pyridoxine): Benefits, Dosage & Deficiency Guide
Vitamin B6 is a cofactor in more than 100 enzymatic reactions, more than almost any other vitamin, spanning amino acid metabolism, neurotransmitter synthesis, and red blood cell formation. What It Actually Does
PLP is required for the synthesis of serotonin, dopamine, GABA, and norepinephrine — the core neurotransmitters implicated in mood, motivation, and stress regulation. It's also essential for converting the amino acid homocysteine into cysteine, working alongside folate and B12 in that pathway; elevated homocysteine is an independent risk marker for cardiovascular disease, which is why B6 status is sometimes measured alongside those other B vitamins in cardiovascular risk workups. Additionally, B6 is required for hemoglobin synthesis, meaning a deficiency can produce a specific type of anemia even when iron levels are normal.
Deficiency Signs
True B6 deficiency is uncommon in developed countries but occurs more frequently in older adults, people with alcohol use disorder, and those with malabsorption conditions like celiac or Crohn's disease. Certain medications also deplete B6, most notably isoniazid (used for tuberculosis) and some long-term users of oral contraceptives or the Parkinson's drug levodopa. The Toxicity Paradox

Form Matters: Pyridoxine vs. P5P
Most over-the-counter B6 supplements use pyridoxine hydrochloride, the synthetic, stable form that the liver converts to active PLP. A smaller category of products supply pyridoxal 5'-phosphate (P5P) directly — the already-active coenzyme form. In theory, P5P bypasses a conversion step, which may matter for people with impaired liver conversion capacity, but for most healthy adults, the clinical difference between the two forms in typical supplemental doses hasn't been convincingly demonstrated in controlled trials.
