Vitamin B12 Deficiency: Signs, Causes, and How to Fix It
Vitamin B12 deficiency is one of the most under-diagnosed nutrient problems in adults, largely because its early symptoms — fatigue, brain fog, irritability, tingling in the hands or feet — overlap with dozens of other conditions. A 2000 study in the American Journal of Clinical Nutrition found that up to 39 percent of the U.S. population has "marginal" B12 status, and the deficiency becomes more common with age: an estimated 10–15 percent of adults over 60 are clinically deficient, according to a 2014 review in the Annual Review of Nutrition.
Why It Gets Missed
B12 is stored in the liver in large enough quantities that a true deficiency can take years to develop after intake drops or absorption fails. Standard blood panels often don't include B12 unless specifically requested, and the commonly used serum B12 test has known blind spots — a 2006 paper in Blood found that measuring methylmalonic acid (MMA) and homocysteine alongside serum B12 catches functional deficiencies that a standard test alone misses.
Who's At Risk
B12 is found almost exclusively in animal products, so vegans and vegetarians are at elevated risk without supplementation. Absorption also declines with age because it depends on stomach acid and a protein called intrinsic factor, both of which decrease over time — this is why B12 deficiency is disproportionately common in people over 50, independent of diet. People taking metformin or long-term proton pump inhibitors, and anyone with Crohn's disease, celiac disease, or a history of gastric bypass surgery, face higher risk due to impaired absorption rather than low intake.
The Symptoms Worth Taking Seriously
Early signs are nonspecific: fatigue, low mood, poor concentration. Left uncorrected, deficiency can progress to peripheral neuropathy — numbness or tingling in the extremities — and in advanced cases, irreversible neurological damage. A 1988 study in the New England Journal of Medicine documented neurological symptoms appearing in some patients before any anemia showed up on bloodwork, which is why B12 deficiency can't be ruled out just because a standard complete blood count looks normal.
Cyanocobalamin vs. Methylcobalamin
Most inexpensive supplements use cyanocobalamin, a synthetic form the body must convert into active methylcobalamin or adenosylcobalamin before it can be used. This conversion is inefficient in some people, particularly those with certain MTHFR gene variants. Dosing

A 1998 study in the British Medical Journal found that oral doses of 1000–2000 mcg daily were as effective as injections for correcting deficiency in most patients, injections generally reserved for severe deficiency or absorption disorders like pernicious anemia. Testing Before Supplementing
Because deficiency symptoms are so nonspecific, testing before and roughly three months after starting supplementation is the only reliable way to know it's working. Ask for serum B12 plus MMA if levels are borderline. Retesting also matters for people on high-dose regimens, since some who supplement heavily still show poor correction due to unaddressed absorption issues that supplementation alone won't fix.
