Vitamin A: Why Deficiency Is More Common Than You'd Think

The World Health Organization has estimated that vitamin A deficiency affects roughly 190 million preschool-age children worldwide, and it's treated as a developing-world problem. But subclinical, low-grade vitamin A insufficiency shows up in wealthy countries too — driven not by lack of food but by fat malabsorption, restrictive dieting, and a genetic variant that quietly blocks conversion from plant sources. Two Different Nutrients Called "Vitamin A"

Who Is Actually at Risk

In a food-secure country, clinical deficiency is rare, but several groups run a real risk of insufficiency: people on very low-fat diets (vitamin A is fat-soluble and needs dietary fat for absorption), anyone with a fat-malabsorption condition such as Crohn's disease, celiac disease, or pancreatic insufficiency, bariatric surgery patients whose altered digestive anatomy reduces absorption, and people with heavy, sustained alcohol use, since the liver both stores and mobilizes vitamin A and chronic alcohol exposure damages both functions.

The earliest and most specific symptom is night blindness — difficulty adjusting to low light because retinol is required to regenerate rhodopsin, the visual pigment in rod cells. Left uncorrected, deficiency progresses to dry eyes (xerophthalmia), rough, bumpy skin from follicular hyperkeratosis, and a measurably higher rate of infection, since vitamin A maintains the mucosal barriers in the respiratory and gastrointestinal tract and regulates T-cell differentiation in the immune system.

The RDA and Why It's Reported the Way It Is

The current recommended intake is 900 mcg RAE (Retinol Activity Equivalents) per day for adult men and 700 mcg RAE for adult women. RAE exists specifically because retinol and beta-carotene aren't interchangeable milligram-for-milligram — it takes roughly 12 mcg of dietary beta-carotene to yield 1 mcg of retinol activity, which is why old IU-based labeling routinely overstated how much usable vitamin A a plant-based product actually delivered.

The Toxicity Ceiling Matters More Than the RDA

Unlike water-soluble vitamins, vitamin A is stored in the liver and doesn't get flushed out when you overshoot. Chronic intake above that threshold is associated with liver damage and reduced bone density, and acute megadoses can cause symptoms ranging from headache and blurred vision to intracranial pressure changes. This ceiling doesn't apply to beta-carotene, which the body converts to retinol only as needed and doesn't accumulate to toxic levels the same way.

This distinction matters most for pregnant women: the American College of Obstetricians and Gynecologists advises against high-dose preformed vitamin A supplements during pregnancy because of documented teratogenic risk, while beta-carotene from food carries no such warning. If you're pregnant or planning to be, check exactly which form is in your prenatal — "vitamin A" and "retinol" on a label are not the same risk category as "beta-carotene."

Getting It From Food First

Beef liver is astonishingly concentrated — a single 3-ounce serving can exceed the entire daily RAE requirement several times over, which is itself a reason not to eat it daily alongside a supplement. Eggs, full-fat dairy, and salmon provide moderate preformed amounts.
