Iodine and Thyroid Health: What the Research Actually Shows

Iodine gets almost no attention in wellness culture compared to magnesium, vitamin D, or omega-3s — yet it's the single mineral your thyroid cannot function without. The World Health Organization estimates that roughly two billion people worldwide have insufficient iodine intake, and much of that isn't happening in developing nations. It's happening in kitchens that quietly moved away from iodized salt.

Why the Thyroid Needs It

The thyroid gland uses iodine as the literal building block for thyroxine (T4) and triiodothyronine (T3) — the hormones that set your metabolic rate, regulate body temperature, and support brain development in children. No iodine, no hormone synthesis, regardless of how well the gland itself is functioning. When intake drops too low, the pituitary gland raises TSH to compensate, and the thyroid can enlarge in response — the mechanism behind goiter, one of the oldest recognized iodine deficiency disorders.

How Deficiency Crept Back In

Iodized salt programs, introduced in the United States in the 1920s, largely solved deficiency for decades. But NHANES data published by Hollowell and colleagues in the Journal of Clinical Endocrinology & Metabolism found that median urinary iodine concentration in the U.S. population fell by about 50 percent between the early 1970s and the early 1990s — driven by lower salt intake overall, a shift toward processed foods that use non-iodized salt, and dairy industry changes that reduced iodine content in milk. Levels have stabilized since, but a meaningful share of reproductive-age women in the U.S. remain mildly deficient by current biomarkers.

Pregnancy Is Where the Stakes Are Highest

A 2013 study in The Lancet by Bath and colleagues, using data from the Avon Longitudinal Study of Parents and Children (ALSPAC), found that mild-to-moderate iodine deficiency during pregnancy was associated with lower child IQ and reading scores at age eight and nine. The developing fetal brain depends entirely on maternal thyroid hormone in early pregnancy, before the fetal thyroid becomes active around week 18–20. This is why the NIH Office of Dietary Supplements sets a substantially higher RDA for pregnancy (220mcg/day) and lactation (290mcg/day) compared to the standard adult RDA of 150mcg/day.

The Part Wellness Content Usually Skips: You Can Get Too Much

Unlike magnesium or vitamin D, iodine has a narrow therapeutic window, and more is not automatically better. A landmark study by Teng and colleagues published in the New England Journal of Medicine (2006) tracked Chinese communities with different iodine intake levels and found that both insufficient and excessive iodine intake were associated with higher rates of thyroid dysfunction, including subclinical hypothyroidism and autoimmune thyroiditis. Very high acute iodine intake can trigger the Wolff-Chaikoff effect — a temporary shutdown of thyroid hormone production — and in susceptible individuals, chronic high intake is linked to Hashimoto's-pattern autoimmunity. The NIH sets a tolerable upper intake level of 1,100mcg/day for adults; some high-dose kelp and iodine protocols marketed online exceed that by several multiples.

If you have any personal or family history of thyroid disease, talk to a doctor and get thyroid antibodies and TSH checked before supplementing — this is one supplement category where self-experimentation carries real downside. Books like The Iodine Crisis are a useful entry point into the history of the deficiency debate, but they represent one side of an active clinical disagreement, not settled consensus.

Food Sources and Practical Dosing

Sea vegetables (kelp, nori, wakame), dairy, eggs, and cod are the most reliable dietary sources. A quarter teaspoon of iodized salt supplies roughly 71mcg. For most healthy adults without thyroid disease, a moderate daily supplement in the 150–300mcg range — such as Life Extension Sea-Iodine or a kelp-based tablet like NOW Foods Kelp — stays comfortably within RDA-to-upper-limit range without approaching megadose territory. The Bottom Line

Iodine is not a mineral to ignore, but it's also not one to megadose on faith. The research supports correcting a genuine deficiency — confirmed through diet review or, ideally, a urinary iodine test — rather than assuming more is protective. For thyroid health specifically, adequate iodine matters, but so does selenium, which supports the enzymes that convert T4 to active T3; the two nutrients work as a pair, not in isolation.
