Grip Strength: The Overlooked Biomarker for Longevity and Performance
In 2015, the Prospective Urban Rural Epidemiology (PURE) study published in The Lancet followed roughly 140,000 adults across 17 countries for four years and measured grip strength with a simple handheld dynamometer. The finding that got researchers' attention: for every 5-kilogram drop in grip strength, all-cause mortality risk rose by 16 percent, and grip strength predicted death from any cause and from cardiovascular disease more strongly than systolic blood pressure did.
Why a Hand Squeeze Predicts Mortality
Grip strength isn't really about hands. It's a proxy for total-body muscular function, and total-body muscular function is entangled with nearly every system researchers care about in aging: skeletal muscle mass, neuromuscular signaling efficiency, nutritional status, and systemic inflammation. Forearm and hand musculature responds to the same anabolic and catabolic signals as the rest of the body, so when muscle wasting begins — from age, inactivity, or illness — grip strength tends to decline early and measurably, making it a cheap, fast surrogate for a much harder question: how much functional muscle does this person actually have.
This isn't a one-off finding. A 2007 study in the BMJ using data from the English Longitudinal Study of Ageing found that midlife grip strength predicted mortality and disability more than a decade later, independent of other known risk factors. Later work using UK Biobank data — over 500,000 participants — replicated the association between low grip strength and higher risk of cardiovascular disease, respiratory disease, and all-cause mortality, and found the relationship held even after adjusting for physical activity levels.
What Grip Strength Is Actually Measuring
Sarcopenia — the age-related loss of skeletal muscle mass and function — typically begins in the fourth decade of life and accelerates after 60. Because it's difficult and expensive to measure muscle mass directly (DEXA scans, MRI), grip strength has become one of the diagnostic criteria used by clinical bodies like the European Working Group on Sarcopenia in Older People. A weak grip is one of the fastest, cheapest ways to flag someone at risk before the broader consequences — falls, fractures, loss of independence — show up.
Outside of aging research, grip strength is also a hard limiter in sport. In deadlifts, pull-ups, rock climbing, judo, and wrestling, the hands frequently fail before the target muscle group does. Athletes training grip specifically often see carryover gains in these lifts simply by removing the grip bottleneck, not because the target muscle got meaningfully stronger.
How to Train It
Grip strength responds to the same principles as any other muscular adaptation: progressive overload, applied through a mix of crush, pinch, and support grip work.
Dead hangs
Hang from a pull-up bar with straight arms for time. This trains support grip — the ability to hold a load passively — and doubles as a light shoulder-decompression exercise. Start with three sets of 20–30 seconds and build toward one to two minutes total time under tension per session.
Farmer's carries
Walk for distance or time while holding a heavy weight in each hand. This trains support grip under load and total-body stability simultaneously, and transfers directly to real-world carrying tasks.
Crush grip training

Thick-bar and pinch training

Rough population benchmarks from the research: healthy adult men typically produce 35–55 kg of grip force, women 20–35 kg, with meaningful decline usually visible after age 50 in untrained individuals. These numbers vary by measurement protocol and population, so treat them as a general orientation rather than a diagnostic threshold — the trend over time matters more than any single reading.
Progression Over 8 Weeks
A reasonable starting structure: two sessions per week combining one crush-grip exercise, one support-grip exercise (dead hangs or farmer's carries), and one pinch or thick-bar movement, for three sets each. Add resistance or duration every one to two weeks as the current load becomes manageable. Grip muscles recover faster than large muscle groups, so two to three sessions a week is sustainable without the overtraining risk that comes with heavier compound lifts.
