Folate (Vitamin B9): Deficiency Signs, MTHFR, and What Actually Works

Folate deficiency is one of the few nutrient gaps that shows up directly in a standard complete blood count — as macrocytic anemia, where red blood cells are produced larger and less functional than normal. But by the time bloodwork catches it, the deficiency has usually been building for months, quietly affecting mood, cognition, and cardiovascular markers well before red blood cell size changes. Folate (vitamin B9) is required for DNA synthesis, red blood cell formation, and the methylation cycle that governs neurotransmitter production — and a meaningful share of the population can't process the synthetic form found in most fortified food and cheap supplements as efficiently as assumed.

Folate vs. Folic Acid: Not Interchangeable

Folate is the naturally occurring form found in leafy greens, legumes, and liver. Folic acid is the synthetic, oxidized form used in fortified flour and most multivitamins because it's cheaper to produce and more heat-stable. The body has to convert folic acid through several enzymatic steps — the last of which is catalyzed by the enzyme MTHFR (methylenetetrahydrofolate reductase) — into L-5-methyltetrahydrofolate (5-MTHF), the form your cells actually use.

An estimated 30–40 percent of people carry at least one variant of the MTHFR C677T gene that reduces enzyme efficiency, and 10–15 percent carry two copies, which research published in the American Journal of Clinical Nutrition has associated with reduced folic acid conversion capacity of up to 70 percent. For these individuals, high folic acid intake without adequate conversion can leave unmetabolized folic acid circulating in the blood — a pattern some researchers have linked to masking B12 deficiency symptoms and altered natural killer cell activity, though the clinical significance of the latter is still debated.

Deficiency Signs

Early folate deficiency is nonspecific — fatigue, irritability, and difficulty concentrating are the most commonly reported symptoms, which is exactly why it's easy to miss or misattribute to stress. As deficiency progresses, glossitis (a swollen, smooth tongue), mouth ulcers, and macrocytic anemia become detectable. Because folate and vitamin B12 both feed into the same methylation pathway, B12 status should always be checked alongside folate — correcting one while ignoring a B12 deficiency can worsen neurological symptoms.

People at elevated risk include pregnant women (folate is critical for neural tube development in the first four weeks of pregnancy, often before a pregnancy is confirmed), anyone with celiac disease or inflammatory bowel disease affecting small intestine absorption, heavy alcohol users, and people on methotrexate or certain anticonvulsants that interfere with folate metabolism.

Homocysteine and Cardiovascular Health

Folate, along with B6 and B12, is required to convert homocysteine — an amino acid byproduct — back into methionine. When folate status is low, homocysteine accumulates. Elevated homocysteine has been consistently associated with increased cardiovascular risk in observational studies, and a meta-analysis in The Lancet found that folic acid supplementation lowers homocysteine levels by roughly 25 percent on average. Whether lowering homocysteine directly reduces cardiovascular events, rather than simply correlating with better overall nutritional status, remains an active area of research — the supplementation trials have shown more consistent benefit for stroke risk reduction than for heart attack risk.

Should You Take Methylfolate Instead?

For people with known or suspected MTHFR variants, or anyone who has tried folic acid without symptom improvement, supplementing directly with L-5-methyltetrahydrofolate (5-MTHF) bypasses the conversion bottleneck entirely. It's the bioidentical, ready-to-use form, and unlike folic acid it doesn't accumulate unmetabolized in the bloodstream at typical doses. The tradeoff is cost — methylfolate supplements run several times the price of standard folic acid.

You don't need genetic testing to justify trying it; the practical signal is simpler: if folate-rich foods and folic acid haven't moved the needle on fatigue or bloodwork, methylfolate is the next reasonable step, ideally alongside a B12 check.

Dosing

The RDA for adults is 400 mcg DFE (dietary folate equivalents) daily, rising to 600 mcg during pregnancy and 500 mcg while breastfeeding. Most methylfolate supplements are formulated between 400 and 1,700 mcg DFE per serving. There's no established benefit to megadosing folate — the tolerable upper limit for synthetic folic acid is 1,000 mcg/day for adults specifically because of the unmetabolized folic acid concern, though this ceiling doesn't apply to natural folate or methylfolate from food or supplements in the same way.
