Compression Garments for Recovery: What the Research Actually Shows
Graduated compression socks and sleeves went from a hospital-supply item for preventing deep vein thrombosis to a fixture of marathon expos and CrossFit gyms in under two decades. The pitch is consistent across brands: better blood flow, less swelling, faster recovery. The research is more measured than the marketing, and the size of the benefit — where one exists — depends heavily on what outcome you're actually asking about.
The Proposed Mechanism
Graduated compression applies more pressure at the ankle or wrist and progressively less further up the limb, which is theorized to assist venous return — the flow of blood back toward the heart against gravity — and to reduce the muscle oscillation (the wobble of muscle tissue on impact) that some researchers believe contributes to exercise-induced muscle damage. A secondary mechanism is simple mechanical support for swelling: compression counters the fluid buildup in tissue that occurs after intense eccentric loading, such as downhill running or heavy lifting.
What the Recovery Evidence Shows
A widely cited 2014 meta-analysis in the British Journal of Sports Medicine (Hill et al.) pooled multiple trials and found that compression garments produced a small-to-moderate reduction in delayed-onset muscle soreness (DOMS) and a moderate reduction in muscle swelling in the 24–96 hours following exercise, with negligible effect on the return of muscle strength or power. A 2016 systematic review by Marqués-Jiménez and colleagues reached a similar conclusion: modest reductions in perceived soreness and creatine kinase (a blood marker of muscle damage), but no consistent effect on actual performance recovery — meaning you may feel less sore without necessarily performing better sooner.
What the Performance Evidence Shows
For wearing compression during exercise itself, the picture is weaker still. A 2013 review in Sports Medicine (Born et al.) examining compression garments and running, cycling, and team-sport performance found minimal or no effect on measures like VO2 max, running economy, or time-trial performance. Where athletes do report feeling better — less muscle vibration, a subjective sense of support — the effect appears to be as much perceptual and proprioceptive as physiological. That's not nothing; perceived exertion genuinely affects pacing and effort tolerance, but it's a different claim than "improves your race time."
Pressure and Fit Actually Matter
Much of the inconsistency across studies traces back to garment quality. True graduated compression in the 20–30mmHg range, correctly sized to calf or thigh circumference (not shoe size, which many cheaper size charts default to), is what the positive recovery trials used. Loose, non-graduated, or novelty "compression" apparel without a specified mmHg rating is unlikely to replicate those results — it's providing coverage, not gradient pressure. Who It's Actually Worth It For
The clearest use case is endurance athletes training or racing on consecutive days — marathon travel, stage races, back-to-back tournament matches — where reduced perceived soreness and swelling between sessions has practical value even without a strength-recovery effect. Long-haul travel is a second legitimate use, since graduated compression has solid evidence for reducing lower-limb swelling and DVT risk on flights, independent of the sports context entirely. It is not, based on current evidence, a substitute for sleep, protein intake, or adequate rest between hard training blocks — those interventions have far larger effect sizes on actual recovery.
Choosing a Garment
For lower-leg recovery and travel, calf compression sleeves are the simplest entry point and easiest to size correctly.
