Choline: The Essential Nutrient Most People Are Missing
Choline wasn't officially classified as an essential nutrient until 1998, when the Institute of Medicine set adequate intake levels for the first time. It's a strange gap in nutrition science, because choline underpins cell membrane structure, liver fat transport, and the synthesis of acetylcholine — the neurotransmitter responsible for muscle control, memory, and attention. Despite that, surveys using NHANES dietary data consistently find that more than 90 percent of American adults fall short of the adequate intake for choline. This isn't a niche deficiency. It's close to universal.
What Choline Actually Does
Choline serves three distinct biological roles. First, it's a structural component of phosphatidylcholine, the dominant phospholipid in cell membranes throughout the body. Second, it's the direct precursor to acetylcholine, a neurotransmitter involved in memory formation, muscle signaling, and parasympathetic nervous system function. Third, choline is a methyl donor — through its metabolite betaine, it participates in the same one-carbon metabolism pathway as folate and B12, helping convert homocysteine back to methionine.
The Liver Connection
The clearest evidence for choline's necessity comes from liver research. Without adequate choline, the liver can't package fat into very-low-density lipoproteins (VLDL) for export, so fat accumulates in liver cells instead. Steven Zeisel's research group at UNC has run controlled feeding studies where healthy adults on choline-deficient diets developed measurable liver damage — including elevated liver enzymes and, in some cases, fatty liver changes visible on imaging — within as little as three weeks. The damage reversed once choline was reintroduced. This is one of the few nutrients where a controlled human deficiency study produced organ damage in under a month.
Pregnancy and Brain Development
Choline demand rises sharply during pregnancy because the nutrient crosses the placenta and supports fetal hippocampal development. Research led by Zeisel and colleagues at Cornell found that maternal choline intake during the third trimester was associated with measurable differences in infant information-processing speed and attention at one year of age. Current guidance from the American Medical Association has called for choline to be added to prenatal vitamin formulations, since most prenatal multivitamins historically excluded it — bulk B-vitamins and folate are cheap and small to formulate into a pill; choline requires a much larger dose to be meaningful, so it's often left out.
The TMAO Question
Choline has picked up a reputation problem because gut bacteria can convert it into trimethylamine (TMA), which the liver oxidizes into TMAO — a compound some observational studies have linked to cardiovascular risk. This finding gets cited often, but it deserves context. TMAO levels are driven heavily by individual gut microbiome composition, and fish — broadly considered heart-healthy — is naturally high in TMAO already, which complicates any simple causal story. Interventional trial data directly testing choline supplementation against cardiovascular outcomes is sparse, and no major medical body currently recommends restricting dietary choline because of TMAO concerns. The nutrient deficiency risk is well-documented and immediate; the TMAO cardiovascular risk is associative and unresolved.
Food Sources and the Supplementation Gap
Egg yolks are the most concentrated common food source — a single large egg provides roughly 147mg of choline, almost entirely in the yolk. Beef liver, salmon, and soybeans are next. The adequate intake set by the Institute of Medicine is 550mg per day for adult men and 425mg for adult women, rising to 450mg during pregnancy and 550mg during lactation. Hitting that target through diet alone typically requires 3–4 whole eggs a day or regular organ meat consumption, which is why so much of the population misses the mark — especially anyone avoiding egg yolks specifically for cholesterol reasons, a practice that's fallen out of favor in more recent dietary guidance.

Dosing and Form Selection
For general adequate-intake purposes, choline bitartrate or a standard choline supplement in the 250–500mg range covers the gap most diets leave. For cognitive-specific goals — focus, memory support, or use alongside nootropic stacks — citicoline in the 250mg range, taken once daily, has the strongest research base for attention and processing speed in both healthy and clinical populations. Sunflower lecithin is a gentler, food-based option that also supplies phosphatidylcholine directly, useful for anyone who prefers to avoid isolated compounds.
