Calcium: What the Research Actually Says About Dose, Form, and Risk

Calcium gets treated as a settled question — take it for your bones, done. The actual research is messier than that. Ninety-nine percent of the body's calcium sits in bone and teeth, and peak bone mass is reached by around age 30, after which the skeleton slowly becomes a withdrawal account. But how much calcium to take, in what form, and whether supplementing it carries cardiovascular risk are all genuinely contested questions in the literature — not settled ones.

How Much You Actually Need

The National Institutes of Health sets the RDA at 1,000mg per day for most adults, rising to 1,200mg for women over 50 and men over 70, when absorption efficiency declines and bone turnover accelerates. Most people get 500–700mg from food alone through dairy, fortified plant milks, and leafy vegetables — the gap is usually smaller than supplement marketing implies, which matters because more isn't automatically better here.

Carbonate vs. Citrate

Calcium carbonate is the most common and cheapest form, and it's roughly 40% elemental calcium by weight. The catch is that it requires stomach acid to dissolve, so it should be taken with food, and it's less reliably absorbed in people over 60 or anyone on long-term acid-reducing medication (PPIs, H2 blockers).

Calcium citrate is about 21% elemental calcium — you need a larger pill for the same dose — but it doesn't depend on stomach acid, so it absorbs consistently whether taken with or without food. The Cardiovascular Question

This is the part most supplement marketing skips. A 2010 meta-analysis in BMJ led by Bolland and colleagues found that calcium supplements without co-administered vitamin D were associated with a roughly 30% increased risk of myocardial infarction across pooled trial data. The proposed mechanism is that a large supplemental bolus spikes serum calcium far above what food intake ever produces, potentially accelerating vascular calcification.

The finding has been disputed — a 2016 review by the National Osteoporosis Foundation and American Society for Preventive Cardiology found the evidence insufficient to establish causation, and dietary calcium at the same total intake shows no such association. The most defensible reading: get calcium from food where possible, and if supplementing, favor moderate divided doses (500mg or less per dose, since the body can't absorb much more than that at once) taken alongside vitamin D and K2 rather than a single large bolus.

Calcium Needs a Team

Vitamin D is required for calcium to be absorbed from the gut at all — without adequate D status, increasing calcium intake does little. The Women's Health Initiative trial found that calcium plus vitamin D together modestly reduced hip fracture risk in postmenopausal women, while calcium alone showed weaker effects. Vitamin K2 plays a separate role, activating proteins that direct calcium into bone rather than soft tissue and arterial walls — it's the piece most supplements leave out entirely.

Practical Guidance

Prioritize food sources first: dairy, canned salmon or sardines with bones, fortified tofu, and leafy greens low in oxalate (kale and bok choy absorb better than spinach, where oxalates bind much of the calcium).
